Physical Therapist

Job Description

OVERVIEW

We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will provide exceptional patient care and support in a dynamic home health setting. As a Physical Therapist, you will play a vital role in helping patients recover from injuries or illnesses and achieve optimal health and wellness.


RESPONSIBILITIES

  • Conduct thorough patient evaluations to identify treatment goals and develop personalized plans to achieve them.
  • Utilize a variety of physical therapy techniques, including manual therapy, exercise programs, and modalities, to promote healing and rehabilitation.
  • Provide patient education on proper body mechanics, posture, and self-care techniques to facilitate recovery.
  • Collaborate with medical professionals to ensure comprehensive care and facilitate referrals.
  • Document patient progress and communicate with patients and families throughout the treatment process.
  • Demonstrate a commitment to patient care and adhere to hospital policies and procedures.
  • Participate in hospital quality improvement initiatives to improve patient outcomes.


QUALIFICATIONS

  • Bachelor's degree in Physical Therapy from an accredited institution.
  • Current licensure to practice as a Physical Therapist in the state of employment.
  • Strong knowledge of medical terminology and anatomy.
  • Experience working in a hospital setting or acute care environment.
  • Ability to work effectively with patients of all ages, including pediatric and geriatric populations.
  • Excellent communication and interpersonal skills.
  • Experience in home community care or outpatient settings is a plus.
  • Proven ability to provide high-quality patient care and meet the needs of diverse patient populations.
  • Familiarity with hospital electronic medical records systems and ability to learn new software applications quickly.


Work Environment: Works indoors in Agency office and patient homes, and travels to/from patient homes.

Job Type: Contract, Per diem

Pay: $70.00 - $90.00 per hour

  • Bachelor's (Required)

  • Hospital experience: 1 year (Required)
  • Patient care: 1 year (Required)
  • Home & community care: 1 year (Required)

Work Location: On the Road
