OVERVIEW
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will provide exceptional patient care and support in a dynamic home health setting. As a Physical Therapist, you will play a vital role in helping patients recover from injuries or illnesses and achieve optimal health and wellness.
RESPONSIBILITIES
- Conduct thorough patient evaluations to identify treatment goals and develop personalized plans to achieve them.
- Utilize a variety of physical therapy techniques, including manual therapy, exercise programs, and modalities, to promote healing and rehabilitation.
- Provide patient education on proper body mechanics, posture, and self-care techniques to facilitate recovery.
- Collaborate with medical professionals to ensure comprehensive care and facilitate referrals.
- Document patient progress and communicate with patients and families throughout the treatment process.
- Demonstrate a commitment to patient care and adhere to hospital policies and procedures.
- Participate in hospital quality improvement initiatives to improve patient outcomes.
QUALIFICATIONS
- Bachelor's degree in Physical Therapy from an accredited institution.
- Current licensure to practice as a Physical Therapist in the state of employment.
- Strong knowledge of medical terminology and anatomy.
- Experience working in a hospital setting or acute care environment.
- Ability to work effectively with patients of all ages, including pediatric and geriatric populations.
- Excellent communication and interpersonal skills.
- Experience in home community care or outpatient settings is a plus.
- Proven ability to provide high-quality patient care and meet the needs of diverse patient populations.
- Familiarity with hospital electronic medical records systems and ability to learn new software applications quickly.
Work Environment: Works indoors in Agency office and patient homes, and travels to/from patient homes.
Job Type: Contract, Per diem
Pay: $70.00 - $90.00 per hour
